Submitting a Request for a JSM-related Section Award
Most sections offer an awards program such as student JSM travel awards, distinguished achievement awards, and JSM paper competitions. Because there are so many, we require you to submit an award request using an online form.
Most awards involve a cash prize, so only section chairs or treasurers can access and complete the online form and authorize payment. Section chairs or treasurers must first log in. If you are not currently logged in, click on the “Login to see members-only content” link found on the upper, right hand, side of this page. Then, return to this page and click on the link at the bottom to begin.
If a cash award is included, you must specify the amount and have the award recipient FAX a completed W9 (or W-8BEN if appropriate) to the attention of Rochelle Edwards at 703.997.7299 before the check can be processed.
Please do NOT email the W9 form since it contains social security information. Regular mail is certainly acceptable at 732 North Washington Street, Alexandria, VA 22314.
For certificates and award checks we need to know if they are presented at JSM or mailed before JSM. If mailed before JSM, requests must be made 30 days prior to the date needed. If presented at JSM, requests must be submitted by June 1.
No IRS W-9 form is required for JSM travel reimbursement. However, the award recipient must submit their travel receipts and ASA Travel Reimbursement Form to the ASA office no later than 30 days after the conclusion of JSM to be reimbursed.
