    Does anybody  know a PhD program combined Biostatistics and Epidemiology? As far a i know there is one in Kentucky.

    We have a PhD in EPI/BST at the University of Kentucky.

    Doctor of Philosophy in Epidemiology and Biostatistics

    There is another at Case Western Reserve University.

    Dartmouth has a PHD  Quantitative Biomedical Sciences which combines Biostatistics, Epidemiology and Bioinformatics:
